Family Medicine

Family medicine services are provided though Mount Sinai's affiliation with the Institute for Family Health, which runs a network of federally qualified community health centers in New York City and the mid-Hudson valley. All of the Institute’s health centers are accredited by the Joint Commission and recognized as Level 3 patient-centered medical homes by the National Committee on Quality Assurance. The Institute is committed to finding innovative ways to improve care for diabetes, depression, women’s health, HIV and other health priorities in the communities we serve. We are also dedicated to primary and specialty care coordination.

About Family Medicine Services

Mount Sinai’s family medicine services are provided through an affiliation with the Institute for Family Health. At the Institute, we run a network of community health centers that provide family medicine services to patients of all ages. We offer:

  • Insurance enrollment and social services
  • Health care for teens
  • Diabetes care
  • HIV care
  • Home visit services for families
  • WIC
  • Veteran’s initiative
  • Free clinics for the uninsured
  • Health care for the homeless

Many of our health centers are open on weeknights and weekends.
