When you or your loved ones need health care that is compassionate and culturally competent, the Jack Martin Fund Center (JMFC) is here for you. We offer primary and specialty care in a safe and welcoming environment. We also provide supportive services—including social work and nutrition counseling—to help you achieve and maintain the best possible health. To make it easy for you, we offer all our services in one location. Our doctors and staff work together to provide seamless care that meets the individual needs of each patient. JMFC is part of the Institute for Advanced Medicine (IAM).

Our Services

At JMFC, we treat everyone from infants to older adults. We provide primary, mental health, dental, and specialty care. In addition, you can leave your appointment with your medications in hand, thanks to our onsite outpatient pharmacy. If you need highly specialized care, we can also easily refer you to experts in the Mount Sinai Health System.

Services for Families and Children

In addition to the services listed above, we offer services specifically for women, children, and adolescents including:

  • Comprehensive Medical Care
  • Pediatric Social Work Services
  • Therapeutic Support Services for Moms and Families
  • Linkage to Behavioral Health and Pediatric Specialties
  • Support groups for HIV-positive women and for new moms at the Women’s Program
  • Promotion of and support for breast health and gynecologic services

We also work with community-based organizations (CBOs) to further help you and your family. These CBOs can provide a range of additional services, such as food pantries, legal services, and additional types of support groups. The CBOs operate offsite.

Populations of Focus

The Institute of Advanced Medicine started in 1989 as a program for HIV+ people. We now offer primary and specialty care to all communities, regardless of your status. We accept most major commercial insurance plans, as well as Medicaid and Medicare. If you do not have health coverage, we may be able to help you obtain insurance. In addition to providing medical and support services, we offer a variety of special programs for specific populations, including:

  • Women, infants, children, and youth
  • Sexually active young adults
  • People leaving jail or prison
  • Older adults
  • Latinx populations
  • LGBTQ+
  • Individuals with a history of mental health or substance use issues
