For Referring Physicians

To refer a patient to the Mount Sinai Clinical Diabetes Institute, please have the patient call 212-241-3422 (commercial insurance) or 212-659-8551 (Medicaid/Fidelis/Humana) for an appointment, and please fax all relevant lab information and office notes ahead of time to 212-423-0508.

If you would like to fast-track a particularly sick patient, call us directly at 212-241-3422 to speak to one of our physicians. We will do our utmost to accommodate urgent referrals. We will also send you a consult note in a timely fashion after your patient’s visit.

Please note that the physicians at the Mount Sinai Diabetes Center cannot and will not function as primary care physicians and feel strongly about patients’ continuity of care with their established team of physicians.
