Enter the Mount Sinai Transitions of Care Center (TOCC); one the few teams of its type offering a centralized, telephone- and video-based discharge program staffed by Mount Sinai registered nurses to head off potential complications and readmissions.

On June 16-18, 2025, the Mount Sinai Health System invites nurse scientists from across the country to a multiday summit discussing shared challenges and opportunities for this rapidly expanding role.
At this moment, a nurse is at a bedside caring for a loved one; at a computer compiling research findings; at a community site bringing tests to those in need; and in a classroom training tomorrow’s clinicians. Advanced Practice Nursing
Every area of the Mount Sinai Health System now includes advanced practice nurses, such as nurse practitioners and nurse anesthetists. They work in primary care and ambulatory settings as well as specialties such as orthopedics, neurosurgery, surgery, cardiology, and more. In many ways advanced practice nurses function as the glue between the nursing and physician worlds. Through their dedication and training, advanced practice nurses contribute a great deal to promoting the team approach that helps patients achieve the best possible outcomes.
