In the first few months after a stroke, many patients feel unsupported and concerned that they are not receiving the care that they need. The Precision Recovery Stroke program is designed to remotely monitor your health and wellbeing after you leave the hospital. Our Precision Recovery team will check in with you weekly to make sure your recovery is moving in the right direction.

Program Breakdown

This remote patient monitoring program has two main components:

1. Daily symptom reports completed directly from a Smart Device or telephone call using the Precision Recovery application
2. Weekly video or phone calls with a healthcare provider to discuss your recovery and complete a more detailed clinical evaluation.
